Multaq (dronedarone): Drug Safety Communication - Increased Risk of Death or Serious Cardiovascular Events
FDA completed a safety review of Multaq (dronedarone). This review showed that Multaq increased the risk of serious cardiovascular events, including death, when used by patients in permanent atrial fibrillation (AF). The review was based on data from two clinical trials, PALLAS and ATHENA. FDA is providing new information and recommendations for the use of Multaq to manage the potential serious cardiovascular risks with the drug.
The Multaq drug label has been revised with the following changes and recommendations:
Healthcare professionals should not prescribe Multaq to patients with AF who cannot or will not be converted into normal sinus rhythm (permanent AF), because Multaq doubles the rate of cardiovascular death, stroke, and heart failure in such patients.
Healthcare professionals should monitor heart (cardiac) rhythm by electrocardiogram (ECG) at least once every 3 months. If the patient is in AF, Multaq should be stopped or, if clinically indicated, the patient should be cardioverted.
Multaq is indicated to reduce hospitalization for AF in patients in sinus rhythm with a history of non-permanent AF (known as paroxysmal or persistent AF)
Patients prescribed Multaq should receive appropriate antithrombotic therapy.

Affected Models: Powerheart 9300A, 9300E, 9300P, 9390A, and 9390E; CardioVive 92532, 92533; CardioLife 9200G and 9231; GE Responder and Responder Pro; and Nihon-Kohden AEDs
Reason for Recall: The affected AEDs contain a component that may fail unexpectedly due to a defect. If the component were to fail during a rescue attempt, the AED may not deliver defibrillation therapy causing serious adverse health consequences, including death. The unit’s self test may not detect the failure or impending failure of the component.

Bedford Laboratories™Issues Nationwide Voluntary Recall of Cytarabine for Injection, USP 1 Gram per Vial
Bedford Lboratories™ announces a nationwide voluntary recall for the following three lots of Cytarabine for Injection, USP:
Cytarabine for injection, USP 1 gram per viral - NDC #55390-133-01
Lot 2066986 - Exp. Date March 31, 2014
Lot 2111675 - Exp. Date April, 30, 2014
Lot 2131148 - Exp. Date May, 31, 2014
This voluntary market recall is being conducted due to a post-release investigation of the manufacturing area which determined a potential elevated risk of a lack of sterility assurance for these specific lots. To date, there have been no reports of any adverse events for the lots being recalled.
Cytarabine in combination with other approved anticancer drugs is indicated for remission induction in acute non-lymphocytic luekemia of adults and children. it has also been found useful in the treatment acute lymphocytic luekemia and the blast phase of chronic myelocytic luekemia. Intrathecal administration of cytarabine is indicated in the prophylaxis and treatment of meningeal luekemia.

Smiths Medical Bivona Neonatal, Pediatric and Flextend Tracheostomy Tubes: Class 1 Recall - Inadvertent Dislodgement
FDA notified healthcare professionals and medical care organizations about the Class 1 recall of certain lots of these tracheostomy tubes. Difficulty arising from disconnecting accessories from the connectors of the affected tubes may result in excessive force to detach the accessory and the tracheostomy tube may dislodge from the patient. This could lead to serious patient injury or death, especially if no replacement tube is immediately available.
BACKGROUND: The Bivona Pediatric, Neonatal and Flextend tracheostomy tube is intended to provide direct airway access for a tracheostomized patient for up to 29 days. This product is used in health care facilities and home care environment. Lot Numbers 1631477 through 1923406 are being recalled. Norgestimate and Ethinyl Estradiol Tablets: Recall - Packaging Error, Potential for Incorrect Dosing Regimen
Glenmark Generics Inc. issued a nationwide, consumer-level recall of seven (7) lots of Norgestimate and Ethinyl Estradiol Tablets USP (0.18 mg/0.035 mg, 0.215 mg/0.035 mg, 0.25 mg/0.035 mg), because of a packaging error where select blisters were rotated 180 degrees within the card, reversing the weekly tablet orientation and making the lot number and expiry date visible only on the outer pouch. Phenylephrine HCl Injection, USP, 1% 5 mL Vial, (Lot# 0693) recalled.
ISSUE: Recall initiated because some vials exhibit translucent visible particles consistent with glass delamination. Potential adverse events after intravenous administration include damage to blood vessels in the lung, localized swelling, and granuloma formation.
BACKGROUND: Glass delamination can occur with high pH solutions when the surface glass from the vial separates into thin layers, resulting in glass particles with a flaky appearance.

NURSING HINTS CORNER
The drainage collection devices used after surgery can irritate your patient's skin when they are taped on and changed often. To avoid the discomfort of tape, take a 6" piece of stockinette and seal one end to form a bag. Then insert the collection device into the new bag. Attach the bag to the patient's dressing, or else make a gauze belt (can use a large stockinette) to hold the bag in place.
Because the bag stays in place without tape, you can change the collection devices whenever necessary and without any worry about hurting your patient. Louise Sweeten, RN.
